Job Summary
Decentralized unit-based Clinical Pharmacists are valued team members working in patient care areas optimizing patient medication therapies by monitoring drug selection, drug dosage, frequency, food/drug allergies, drug- drug interactions and duplication, medication reconciliation and discharge medications. They are a resource for drug information and education for physicians, nurses, and patients. Also, dose medications under protocols. They also round with Intensivists and Hospitalists playing an active role making recommendations for therapies and changes in therapies.
Responsibilities
- Demonstrates the ability to detect problems with drug therapy regimens and recommends suggestions to physicians or nursing staff to resolve drug therapy problems or enhance patient outcomes.
- Provides medication instruction to patients. Discusses new drug product information or other relevant drug information with pharmacists and technicians, physicians and nurses.
- Provide aminoglycoside and vancomycin dosing and monitoring for all hospital patients. Provide Coumadin dosing and monitoring for all those consulted.
- Write TPN orders for all hospital patients, except neonates, while keeping the physician and dietary apprised of the patient’s needs.
- Is responsible for IV to PO conversions, adjusting drug dosages per renal function and auto-substitutions per policy.
- Performs related duties as required.
Qualifications
- Required:
- Education: Bachelor’s Degree in Pharmacy with 5 years Clinical Experience or Doctorate of Pharmacy is required
- Experience:
- 5 years Clinical Experience for Pharmacist with a Bachelor’s Degree in Pharmacy; OR
- Pharmacist with a Doctorate of Pharmacy who has completed rotations
- Licensure/Certification: South Carolina license required
- Preferred:

- Residency trained and Board certification is preferred
- American Heart Association BLS required, must be obtained within 7 days of hire
- ACLS required within orientation period
