- Identifies patients in need of social work intervention within 24 to 48 hours of admission based on high risk screening criteria.
- Follows up on all referrals made to social work from all sources other than high risk screening within 24 hours of referral.
- Assesses patients to determine the bio-psychosocial factors that impact their medical condition, treatment, recovery, and safe transition from one care environment to another
- Develops intervention plan and discharge plan to meet patient’s/family’s needs.
- Provides social work services to patients in OB and takes weekend on call for OB/emergency department.
- Other duties as assigned.
Provide social work services to patients and families in the acute care setting to meet their medically related social and emotional needs as they impinge on their medical condition, treatment, recovery, and safe transition from one care environment to another using an interdisciplinary approach and coordination of timely discharge planning.
- Required:
- Education: Master’s degree in Social Work
- Experience: One year experience in an acute inpatient hospital setting or equivalent
experience
- Licensure/Certification: LCSW eligible.
- Preferred:
- Licensure/Certification: LICSW preferred
